Next Steps

To work with children in the classroom, California law requires volunteers to supply the following information.

  1. Provide proof of measles and whooping cough (pertussis) vaccination, either via blood test or official medical record.

  2. Provide evidence of flu vaccination or qualified exemption.

  3. Provide a negative tuberculosis test or a TB risk assessment (you can print out this form and bring it to your doctor).  

  4. Fill out and sign the Criminal Record Statement (this is only necessary if you are over 18 years old).

  5. If you are under 18 years old, have a parent or guardian fill out the minor volunteer permission form.
